What is pelvic congestion syndrome (PCS)?
PCS is a condition characterised by persistent pelvic pain lasting six months or more, unrelated to the menstrual cycle or pregnancy. The pain arises when veins in the ovaries and pelvis become enlarged and twisted, leading to increased pressure on surrounding organs. This can contribute to symptoms such as stress incontinence, irritable bowel syndrome, painful periods, backache and fatigue. PCS most commonly affects women aged 20 to 45 who have had more than one pregnancy, but other risk factors include varicose veins and polycystic ovarian syndrome.
How does screening work?
A vascular transvaginal ultrasound is an internal scan that provides a more detailed view of the blood flow through the pelvic organs, helping to detect PCS. The procedure involves gently inserting a small ultrasound probe (similar in size to a tampon), called a transducer, into the vagina. Covered with a sheath and lubricating gel, the transducer emits sound waves that create real-time images of the pelvic organs on a screen. The scan typically takes 15 to 30 minutes.
Treatment options
If PCS is diagnosed, medication is usually offered as a first line of treatment.
If medication does not relieve the pain, a procedure called pelvic vein embolisation may be recommended. This minimally invasive procedure blocks the problematic veins, preventing blood from flowing backward. A consultant uses a catheter to deliver tiny coils, glue or foam into the affected veins, relieving pressure and reducing pain.
